主动脉机械瓣膜置换术后低强度抗凝研究

Research on clinical supervise of anti-coagulation therapy after replacement prosthetic mechanical aortic valve

目的 研究广东地区人工机械主动脉瓣膜置换术后行低强度抗凝的可行性及安全强度。方法 对120例主动脉机械瓣膜置换的患者，按国际标准化比值（INR）进行抗凝监测，其范围控制在1．5～1．8。并对患者应用华法林抗凝治疗的临床效果进行回顾分析。结果 120例患者术后抗凝并发症明显低于文献报道，无栓塞病例，少许轻微出血事件亦无需特殊处理，临床效果良好。结论 主动脉瓣人工机械瓣膜置换术后。在我国南方地区行较低强度抗凝治疗（INR=1．5～1.8），具有较高的安全性和可行性。

Objective To evaluate the possibility and safety bounds of low intense anti-coagulation therapy after replacement prosthetic mechanical aortic valve. Methods 120 cases with replacement prosthetic mechanical aortic valve were studied. According to the international normalized ratio, INR of all the patients were controlled in 1.5 - 1.8. The clinical anticoagulation effect were analyzed to provide or reduce the occurrence of anticoagulation related complications. Results The rates of complications in 120 patients were lower than those reported in the foreign literature. Conclusion It is acceptable to practice low intense anti-coagulatlon therapy after replacement prosthetic of aortic mechanical valve in the south of china. And it is safety and feasible to control INR between 1.5 - 1.8 after the operation.
