30s深呼吸4次法与传统吸氧去氮法比较 被引量:1

摘要 目的:比较全麻插管诱导前传统吸氧去氮法与30s深呼吸4次法的差别。方法:20例气管插管全麻病人随机分为吸氧去氮组(3min组)与30s深呼吸4次组(4DB组),所有病人入手术室后在呼吸空气情况下采集动脉血气标本做血气分析,并记录动脉血氧饱和度(SpO2)值作为基础值。3min组采用完全密闭面罩,氧流量10L,麻醉机活瓣旋开以15次/min频率吸氧3min。4DB组在相同条件下在30s内深呼吸4次。吸氧结束后即刻记录SpO2和采集动脉血做血气分析。随后两组在相同条件下快诱导插管,插管后保持病人无呼吸状态下保持气管导管开放,记录各例病人SpO2降至97%、95%、93%的时间,并在SpO2降至93%时采集动脉血标本测定血气分析。结果:SpO2降至97%、95%、93%各时点的时间3min组均明显长于4DB组(P<0.05)。吸氧后动脉血氧分压(PaO2)值3min组明显高于4DB组(P<0.05)。吸氧后动脉血二氧化碳分压(PaCO2)值两组相近,但SpO2降至93%时3min组PaCO2值明显高于4DB组(P<0.05)。结论:全麻诱导前的给氧方法中,传统吸氧去氮法较30s深呼吸4次法有更多的优点。
