石棉工人肺功能5年前后比较

A 5-Year Before-and After Comparison of Lung Function in Asbestos Workers

为寻找反应石棉工人肺功能损伤的最早、最敏感的指标，作者采用水封式肺量计和弥散功能测定仪对１１９名男性石棉作业工人１９８９年和１９９４年的肺功能进行了测定，并比较其５年前后各项指标的变化情况。其中健康接尘组５０例，石棉肺现在对象２５例，Ⅰ期石棉肺患者３６例，Ⅱ期右棉肺患者８例。结果显示：５年后４组工人的用力肺活量（ＦＶＣ），第一秒呼气量（ＦＥＶ１）和弥散数（ＤＬｃｏ）值均有降低，其中ＦＶＣ和ＤＬｃｏ值与５年前比较有显著性差异（Ｐ＜０．０５）；５年后Ⅱ期石棉肺组ＦＶＣ值下降显著，与其它三组比较有显著性差异（Ｐ＜０．０１），表明Ⅱ期病肺功能下降迅速。作者认为ＤＬｃｏ和ＦＶＣ是反映石棉工人肺功能损伤最早、最敏感的指标。

A 5-year before-and-after comparisonof lung function vvas carried out in 119 male asbestosworkers. Of them 50 were healthy workers(0), 25were patients with suspectable asbestosis(0+),36with Stage Ⅰ asbestosis(Ias) and 8 with Stage Ⅱ as-bes-tosis(Ⅱ as ). The findings measured for beforeand after 5 years revealed that FVC, FEV and DLcodeclined in the four grou ps over the whole period ofobservation. Tbedecrease in FVC and DLco was mostmarked and the differences were statistically signifi-cant (P<0. 05 ).Comparison among four groupsshowed that all indices of lung function in Group Ⅱ asreduced significantly after 5 years (P<0.01), indicat-ing a rapid decrease of lung function in Group Ⅱ as. Itis suggested that FVC and DLco are the most sensitiveindices in detecting very early abnormalities of lungfunction in asbestos workers.
