腹腔镜下膀胱部分切除术

Laparoscopic partial cystectomy

目的探讨腹腔镜下膀胱部分切除术治疗膀胱肿瘤的临床应用价值及可行性。方法采用经腹腔途径对8例膀胱肿瘤患者实施腹腔镜下膀胱部分切除术,手术操作包括膀胱部分切除及完全腔内膀胱缝合。结果8例手术均成功,平均手术时间230min,术中平均失血300mL,术中术后无明显并发症,平均住院时间7d。病理学检查证实均为移行细胞癌。2例分别在术后随诊12个月及14个月时有局部复发及全身转移,给以保守治疗。其他6例在8￣22个月的随诊中尚未发现局部肿瘤复发及远处转移。结论腹腔镜下膀胱部分切除术可行,具有微创、恢复快、住院时间短等优点,是一种可选择的治疗膀胱肿瘤较为理想的手术方法。

[Objective] To evaluate the clinical value and feasibility of laparoscopic partial cystectomy in neoplasms. [Methods] 8 cases of bladder cancer underwent transperitoneal laparoscopic partial cystectomy. The procedure employed consisted in laparoseopic partial cystectomg and exclusive intracorporeal suture technique. [Resuits] The procedure was completed in all cases. Mean surgical time was 230 minutes and mean blood loss was 300 mL. There were no significant complications during both intra-and post-operative period. Mean hospitalization period was seven days. The histological analysis of the resected specimens revealed transitional cell carcinoma. Two patients developed local recurrence and systematic metastases after operative twelve months or fourteen months and was treated with salvage chemotherapy. No other case of local or systemic recurrence was observed during the following eight to twenty-two months. [Conclusions] Laparoseopic partial cystectomy is feasible and has many advantages of minimal invasive, postoperative early recovery, shorter hospital stay, It can be an alternative surgical method for treating patient with bladder neoplasms.
