依达拉奉联合川芎嗪治疗脓毒血症大鼠实验研究 被引量:3

摘要 目的总结大鼠盲肠结扎穿刺法脓毒血症模型的制作和成功标志,探讨依达拉奉联合川芎嗪对脓毒血症大鼠肝功能的保护作用。方法SD大鼠40只分正常组、假手术组、手术对照组、依达拉奉联合川芎嗪治疗组(下称治疗组),每组10只。手术对照组和治疗组予结扎盲肠中部,用16号针头贯通穿孔,制作大鼠脓毒血症模型;治疗组于术前15m in及术后3h各联合腹腔注射依达拉奉5mg/kg和川芎嗪15mg/kg,术后18h采血;假手术组和手术对照组均进行腹水及心脏采血培养,血标本做丙氨酸氨基转移酶(ALT)、天冬门氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)检测。结果模型组腹腔见大量黄色脓性腹水或血性腹水,盲肠变黑、胀气伴恶臭味,手术对照组血培养1只阳性,腹水培养全部阳性,细菌鉴定为大肠杆菌、铜绿假单胞菌、梭形芽胞菌,多为混合感染。正常组、假手术组和手术对照组AST逐渐升高,差异均有统计学意义,假手术组和手术对照组AST比正常组高,P<0.01;治疗组AST较手术对照组明显下降,P=0.038。手术后(包括假手术)大鼠血ALT>100 IU/L,脓毒血症模型成功达91%,AST<100 IU/L者均无脓毒血症表现,P<0.01。结论盲肠结扎穿刺法是比较理想的脓毒血症模型,但不同来源的大鼠要采用不同的操作方式,进行预试,以确定最佳的穿刺孔径。大鼠病态表现、腹腔脓性或血性腹水、腹水细菌培养阳性是模型成功的标志,血ALT>100 IU/L亦可作为模型成功的判断指标。依达拉奉和川芎嗪联合应用能改善脓毒血症大鼠的肝功能。
