电针丰隆穴治疗高脂血症的多中心观察

Multi-center observation of electroacupuncture at Fenglong point in the treatment of hyperlipidemia

目的:观察电针丰隆治疗高脂血症的临床疗效,为丰隆腧穴主治提供临床依据。方法:于2003-09/2005-12选择武汉市中西结合医院针灸科、武汉市中医医院针灸科、华中科技大学同济医学院附属协和医院针灸科收治的高脂血症患者288例为研究对象。随机数字表法分为电针丰隆组144例、药物对照组144例。患者均知情同意。电针丰隆组:取双侧丰隆穴,选用30号2寸华佗牌一次性无菌毫针,以90°快速刺入,进针1.5寸,接LH202型韩氏穴位神经刺激仪,等幅疏密波,电流强度12mA,频率2/100Hz,留针30min,1次/d。连续治疗5次为1个疗程,治疗1个疗程后休息2d,共治疗29d。药物对照组:口服普伐他汀片10mg/次,每晚1次。连续治疗29d。检测治疗前后患者总胆固醇、三酰甘油、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇的变化。结果:3个临床中心共纳入288例病例,247例完成临床研究,其中电针丰隆组127例、药物对照组120例。①药物对照组在调节低密度脂蛋白胆固醇、高密度脂蛋白胆固醇方面明显优于电针丰隆组眼治疗前,治疗结束后2个月药物对照组低密度脂蛋白胆固醇、高密度脂蛋白胆固醇分别为(3.28±0.10),(2.93±0.11)mmol/L;(1.15±0.03),(1.87±0.06)mmol/L;电针丰隆组低密度脂蛋白胆固醇、高密度脂蛋白胆固醇分别为(3.56±0.11),(3.71±0.10)mmol/L;(1.22±0.04),(1.39±0.06)mmol/L,P<0.01演。②两组在调节总胆固醇、三酰甘油方面差异无显著性意义(P>0.05)。③电针和药物在治疗后29d,治疗结束后1,2个月总有效率差异无显著性意义(电针丰隆组分别为56.7%,57.5%,57.5%;药物对照组分别为62.5%,70.8%,74.2%,P>0.05)。结论:电针丰隆组和药物对照组均对血脂有较好的良性调节作用。

AIM： To observe the clinical effect of electroacupuncture at Fengtong point on hyperlipidemia, and-provide clinical evidence for main treatment of points. METHODS： 288 patients with hyperlipidemia were selected from the Department of Acupuncture, Wuhan Chinese ＆ Western Medicine Hospital, Department of Acupuncture, Wuhan Hospital of Traditional Chinese Medicine and the Department of Acupuncture of Wuhan Union Hospital between September 2003 and December 2005. Subjects were randomly divided into electroacupuncture at Fenglong point group （experimental group, n=144） and medicine control group （control group, n=144）. All participants knew and agreed with the items. Patients in the experimental group were acupunctured at bilateral Fenglong points with Huatuo once sterile acupuncture needles of type 30° and 2-cun long）. Needles were vertically stabbed into muscles quickly with the needling of 1.5 tun. And LH2O2 Han＇s Acupoint Nerve Stimulator was adopted with dilatational wave of constant amplitude, The current intensity was 12 mA and the frequency was 2/100 Hz, Needles were kept in muscles for 30 minutes once a day for 5 continuous times as one course. There was an interval of 2 days between each 2 courses and the treatment lasted for 29 days. Patients in the control group orally took pravastatin tablets 10 mg each time and once a day at night for 29 continuous days, The changes of total cholesterol （TC）, triacylglycerol （TG）, low density lipoproteincholesterol （LDL-c） and high density lipoprotein-cholesterol （HDL-c） were examined before and after the treatment. RESULTS： A total of 288 hyperlipidemia cases were enrolled in three clinical centers, and the clinical researches were accomplished on 247 cases, including 127 cases in the electroacupuncture at Fenglong point group and 120 cases in the medicine control group. ①The levels of LDL-c and HDL--c in the control group were significantly down regulated than those in the experimental group [those in the control group before and 2 months after the treatment were respectively （3.28±0.10）, （2.93±0.11） mmol/L; （1.15±0.03）, （1.87±0.06） mmnl/L, and those in the experimental group were （3.56±0.11 ）, （ 3.71 ±0.10） mmol/L; （ 1.22±0.04 ）, （ 1.39±0.06 ） mmol/L, P 〈 0.01 ].②There were no remarkable differences in levels of TC and TG between two groups（P 〉 0.05）.③There were no marked differences in total effective rate between two groups at 29 days, 1 month and 2 months after treatment （those in the experimental group were 56.7% ,57.5% ,57.5% respectively, and those in the control group were 62.5%,70.8%,74.2%,P 〉 0.05 ）. CONCLUSION： The blood lipid in patients of both groups are well regulated.
