晚期贲门癌手术加术中局部化疗远期疗效观察

Long-term Observation of the Effect of the Resection and Intraoperative Local Chemotherapy of Advanced Cardiac Carcinoma

对５６例行肿瘤切除，同时术中局部化疗的晚期贲门癌病人进行了远期疗效观察。Ⅲ期病人４８例，Ⅳ期病人８例。在肿瘤切除同时应用丝裂霉素１０ｍｇ，５－氟尿嘧啶１～２ｇ行食管隔肌裂孔、胰腺被膜、胃左动脉根部及腹主动脉周围、脾门等局部封闭注射，使高浓度的药物长时间发挥作用，起到杀灭残留癌细胞和阻滞其繁殖生长的目的。本组均随访３年以上，１、２、３、年生存率分别为８７．５％（４９／５６）、３０．３６％（１７／５６）、１７．８６％（１０／５６）。

Fifty-six patients with advanced cardiac carcinoma were treated by surgery and intraoperative local chemotherapy in our hospital.of 56 cases,48 were of stage Ⅲ and 8 of stage Ⅳ.While tumors being resected,MMC 10mg and 5-Fu 1￣2g were injected in to hiatus oesophagus,pancreatic diagragm,the root of gastrica sinistra,around aorta abdominalis and lienalis,and so on,which made the dense medicine function long,killing the remaining carcinomatous cells and stopping their reproducing.This group was followed up for over 3 years.The 1-,2-and 3- year survival rates were 87.5 %(49/56),30.36(17/56) and 17.86% (10/56) respectively.
