摘要
目的探讨MUC7和CK20在膀胱移行细胞癌(BTCC)患者尿脱落细胞中的表达及其临床意义。方法采用逆转录聚合酶链反应(RT-PCR)检测42例BTCC患者、40例对照者尿脱落细胞中MUC7和CK20 mRNA表达。结果42例BTCC患者中36例尿脱落细胞中MUC7表达阳性,34例CK20表达阳性,阳性率分别为85.7%和80.9%,40例对照组中有2例MUC7表达阳性,有12例CK20表达阳性,阳性率分别为5.0%和30%;两者诊断敏感性的差异无显著性(P>0.05);两者的诊断特异性分别为95%和70%,MUC7的诊断特异性显著优于CK20(P<0.01)。浸润性BTCC其MUC7的阳性表达率显著高于浅表性BTCC(P<0.05),而CK20在不同临床分期的肿瘤中差异无显著性(P>0.05)。MUC7的表达与BTCC的病理分级呈正相关,CK20的表达在各病理分级间差异无显著性。结论BTCC患者尿脱落细胞中MUC7的阳性表达率明显高于CK20和对照组,尿脱落细胞中MUC7表达与BTCC的临床分期、病理分级有关,而CK20的表达与分期、分级无相关性。
Objective This paper is aimed to study whether it can provide a novel noninvasive tool for the diagnosis of bladder transtional cell carcinoma BTCC the bladder by detection of Muc7 and Ck20 expression in urine using RT-PCR. Methods The morning urine samples of 42 patients with BTCC ,40 persons in control group were collected The expression of MucT,Ck20 and β-actin in exfoliated urinary cells were determined by using RT-PCR.Results Only 2 of 40(5.0% )and 12 of 40(30% ) showed positive expression of MUC7 and CK20 expression in control group. 36 of 42(85.7 % )patients with BTCC showed a positive MUC7 expression and 34 of 42 (80.9 %)showed a positive CK20 expression in urine. The sensitivity in detecting MUC7 and CK20 expression for diagnosis of BTCC is 85.7 % and 80.9%, respectively. Higher specificity was observed in MUG7 expression detection than that in CK20 (95 % vs 70%, P 〈 0.01). Respectively indicated that expression of MUC7 had a positive correlation with the increasing stage of BTCC, But Ck20 expression hadn't this correlation, the expression of MUC7 was positively correlated with the pathologic grading of BTCC while no significant difference was founded in CK20 expression. Conclusion the positive expression rate of MUC7 and CK20 in exfoliated urothclial cell of urine in patients with BTCC is 85.7 % and 80.9 %, respectively, which is significantly higher than that in control group( 5 % and 30 % ) ;MUC7 expression is positively correlated with the staging and grading of BTCC, while CK20 has no this correlation.
