急性心肌梗死QRS波终末变形的预后意义

Prognostic significance of terminal QRS distortion in acute myocardial infarction

目的探讨急性心肌梗死(AMI)的心电图QRS波终末变形与预后的关系。方法回顾性分析181例AMI的首份心电图,根据有无QRS波终末变形,将其分为两组,甲组为变形组58例,乙组为无变形组123例。结果甲组的CPK、CPK-MB的峰值、住院死亡率、心功能Killip≥Ⅲ级的发生率、心律失常发生率均明显高于乙组(P<0.05);甲组的左心室射血分数较低(P<0.05)。结论AMI心电图QRS波终末变形与早期预后有密切关系,可作为负性预后的指标之一。

Objective： To determine the relationship between terminal QRS distortion and prognostic factors in acute myocardial infarction （AMI）. Methods： According to whether the terminal QRS distortion existed （58 cases） or not （ 123 cases）, the initial electrocardiograms of 181cases of AMI were divided into group A and group B respectively. Results： Group A had a higher concentration of peak CPK and CPK - MB, higher incidence of death and Killip class greater than Ⅲ and higher incidence of arrhythmia. Group A had a lower LEVF. Conclusion： The terminal QRS distortion of the initial ECG may have affinity to the early prognosis in AMI, and can be used as one of the factors in predicting the negative prognosis.
