食管癌术中胸导管预防结扎连续1 031例无乳糜胸手术经验

Preventive Ligation of the Thoracic Duct during Esophagectomy: Surgical Experience of 1 031 Chylothorax-free Cases

目的:总结1031例食管癌术中结扎胸导管预防乳糜胸经验。方法:术中于膈上5cm处将胸导管、奇静脉及其间组织一并大块结扎。结果:1031例食管癌切除无术后乳糜胸。结论:采用低位膈上胸导管大块结扎预防食管癌切除术后乳糜胸操作简便,安全可靠。

Objective： To summarize our surgical experience with ligation of the thoracic duct during esophagectomy to prevent chylothorax on 1 031 cases with esophageal carcinoma. Methods： During surgery, the thoracic duct was preventively ligated en bloc with the azygos vein and surrounding tissues at 5cm above the diaphragm. Results： In the 1 031 cases there is no postoperative chylothorax. Conclusion： It is safe and reliable to apply the low-set and supradiaphragmatic en bloc ligation of surrounding tissues with the thoracic duct for prevention of chylothorax following esophagectomy.
