输尿管镜联合钬激光治疗输尿管上段结石

Ureteroscopic Holmium: YAG Laser Lithotripsy for Proximal Ureteral Calculus

目的:总结在输尿管镜联合钬激光治疗输尿管上段结石时防止结石漂移的手术技巧,评价输尿管镜联合钬激光治疗输尿管上段结石的疗效。方法:回顾性分析2002年6月至2005年12月间应用输尿管镜联合钬激光治疗输尿管上段结石共计254例患者临床资料。结果:在应用了本文总结的经验后,2005年碎石单次成功率为94.17%(113/120)与2003年及以前碎石单次成功率84%(42/50)比较,有显著性差异(P<0.05,X2检验)。结论:选择适当病例,合理选择手术时机,提高操作技巧,可以显著提高治疗输尿管上段结石时的单次碎石成功率.

Objective： We summarized the surgery technique of ureteroscopic holmium： YAG laser lithotripsy to avoid stone retropulsion and assessed the effectiveness of ureteroscopic holmium： YAG laser lithotripsy for proximal ureteral calculi. Materials and Methods ： We retrospective analyzed 254 eases of proximal ureteral calculi patients with treatment of ureteroseopic holmium： YAG laser lithotripsy between Jun 2002 and Dec 2005. Results： As skills introduced in this article were performed in 120 patients in 2005, the stone-free rate in the first time therapy was obviously increased to 94. 17% which was significant different to 2003 and before as 84% （P〈0.05, Pearson X2）. Conclusions： The stone-free rate of holmium; YAG laser lithotripsy can be significant increased by experienced urologist if we ehoosed the appropriate patients and surgery opportunities.
