二次甲状腺手术方法探讨

Study of thyroid gland second operation method

目的探讨二次甲状腺手术方法。方法总结分析1993年至2003年84例二次甲状腺手术原因和手术方式。结果由于第一次手术术前诊断为良性肿物而术中或术后诊断为恶性肿物而需要二次手术59例,由于其它良性肿物再发或恶性肿物再发需要二次手术25例。术前误诊为甲状腺腺瘤的32例,结节性甲状腺肿16例,甲状腺囊肿样变8例,甲亢3例,良性肿物再发19例,恶性肿物复发6例。65例行甲状腺根治性切除术,11例行双侧甲状腺大部分切除术,8例行甲状腺腺叶切除术。结论甲状腺疾病的正确诊断和掌握手术方式是甲状腺手术成功的基本条件。

Objective To investigate the methods of thyroid gland second operation. Methods In analyzing and generalizing the causes and methods of 84 cases of second thyroid operation between 1993 and 2003. Results 59 cases needed second operation for malignant tumor which were misdiagnosed as benign tumor in first operation （32 cases adenoma, 16 cases goiter,8 cases thyroid cyst and 3 eases hyperthyroidism） ;25 cases for the postoperative recurrence of benign tumor or malignant tumor （benign tumor recurrence 19 eases and malignant tumor recurrence 6 cases）.65 patients had total thyroidectorny, 11 underwent double subtotal thyroidectomy and 8 had lobeetomy. Conelusion Exact diagnosis and gasping the proper operation methods of thyroid gland are the sueeessful postulate of thyroid operations.
