摘要
目的:探讨肩关节复发性前脱位手术治疗的术式选择的适应征及不同手术方法的临床疗效,以及术后康复治疗的方法。方法:对19例肩关节复发性脱位行手术治疗获得1a以上的术后随访的患者进行回顾总结,15例患者肩关节囊松弛、关节囊无严重破损者行Putti-Platt手术,4例患者肩关节囊前壁严重破损呈碎布条状,无法进行肩关节囊前壁修复和紧缩手术者行Bristow手术,关节功能按优、良、中、差进行评分。结果:本组19例患者术后1年肩关节功能优6例,良11例,中2例。结论:Putti-Platt术式能有效地增强肩关节前方的稳定性,适用于肩关节囊松弛、关节囊无严重破损患者,对于关节囊严重损伤无法进行肩关节囊前壁修复和紧缩的病例可采用操作较为复杂的Bristow手术。术后规范的固定和系统康复训练是保证手术疗效的重要环节。
Objective： To discuss the choice of surgical procedure of recurrent anterior shoulder dislocation and to evaluate its clinical effects. Methods： A total of 19 cases with recurrent anterior shoulder dislocation treated by surgical operation were followed up for at least lyear, 15 cases for Putti - Platt procedure, and 4 cases for Bristow procedure. Results： 6 cases got excellent results, 11 good results and 2 middling results. Conclusion： Putti - platt procedure can strengthen anterior stability of shoulder joint effectively. When the articular capsule were severe destroyed, the more complex surgical treatment as Bristow procedure should be a choice. To get good results after operation, the patient＇s limbs should be freed up normatively and exercised systematically.
