拉下法在中下段直肠癌保肛治疗术中的应用

Application of anus-retained operation for drawing-down the middle and low rectal cancer

回顾分析了1985年6月~2003年6月采用拉下法手术治疗的79例中低直肠癌(距肛缘4~8cm)患者的临床病理资料,结果行拉下法手术后15d^3个月内,患者的肛门括约肌功能逐渐得到恢复,术后半年基本恢复正常。总的局部复发率23·5%(16/68)。1、3、5和10年的生存率分别为98·7%(78/79)、72·2%(37/52)、54·5%(24/44)和47·2%(17/36)。回顾分析结果提示,拉下法手术治疗是中低位直肠癌保肛手术的可行方法之一,适合在基层医院开展应用。

To discussing the effect of Anus-retained Operation for pull-through the middle or low rectal carcinoma. From June 1985 to June 2003 ,seventy-nine patients with distal rectal cancer（the distance to sphincter was 4--8 centimeters） ,were operated by the means of the pull-through. After 15 days to 3 months of the operation ,the function of patient＇s sphincter ani was gradually recovered, and the function was basically come back after half year. The one-year, three-year, five-year and ten-year survival rate （The number of patients is still alive with follow-up study reaching one-year, three-year, five-year and ten-year/The total number of patients with follow-up study reaching one- year, three-year, five-year and ten-year ）was： 98.7% （78/79）,72.2% （37/52）,54.5% （24/44）,47.2% （17/36）. Local recurrence rate was： 22.1% （15/68）. The pull-through operation may be fitter for grass roots hospital ,but it＇ s important to master the indication in order to reduce the local recurrence rate.
