胆总管结扎、胆囊牵拉对家兔左心室功能的影响

Left ventricular responses during iigation of common bile duct and pulling gallbladder

胆总管结扎和胆囊牵拉可以显著影响左心室功能。14只家兔设自身对照组,模拟临床胆总管良性完全性梗阻伴胆红素血症的动物模型,分别在胆总管结扎前、后;胆囊牵拉前、后观察其对左心室功能的影响。结果显示胆总管结扎后左心室肌收缩速度即有明显下降(与结扎前相比,P<0.01)。胆囊牵拉时左心室功能指标全面下降,并伴有动脉压和心率的下降(与牵拉前相比,P<0.05和0.01)。

The experiment was undertaken to investigate the influences of ligation of the common bile duct (CBD) and pulling the gallbladder on the left ventricular function. Fourteen rabbits served as the subjects with the CBD being tied off. The cathter was inserted into the left ventricle to measure left ventricular stroke pressure (LVSP) and±dp/dt, which were recorded before and 5 days after the ligation of CBD respectively. The results showed that the value of +dp/dt decreased significantly (P<0.01) however the value of -dp/dt and LVSP changed unsignificantly 5 days after the ligation of CBD (P>0.05); LVSP, SP, HR and the values of ±dp/dt reduced markedly immediately after the gallbladder being pulled (P<0.05 or 0.01). It is indicated that the depression of left ventricular function may be related to the vagotonia induced by hyperbilirubinemia and the pull at gallbladder.
