摘要
目的探讨细胞表面糖基的变化与外阴表皮细胞良性或恶性增殖的关系。方法采用链霉素抗生物素-过氧化物酶组织化学技术,应用12种凝集素研究60例外阴良、恶性疾病中细胞膜糖基的表达和分布情况。结果从正常外阴表皮到外阴鳞状细胞癌凝集素MPA,ConA,WGA和RCA表达分布无明显的变化。凝集素DBA,LTA和SBA表达均阴性。仅凝集素UEA-1,PNA,VVA,DSA和HPA的表达发生改变。在正常外阴表皮的基底细胞UEA-1,PNA和HPA染色呈阴性,而在外阴鳞状细胞癌出现UEA-1和PNA的阳性表达。在外阴正常表皮VVA和DSA均无表达,但在外阴鳞状细胞增生,外阴尖锐湿疣和外阴鳞状细胞癌中出现不同程度和分布的表达。结论岩澡糖是外阴角朊细胞终末分化的一种标志物;乙酰氨基半乳糖(GalNAc)残基随着外阴表皮细胞的分化而逐渐出现;在外阴细胞恶性转化过程中出现Tn和T抗原的表达;凝集素UEA,VVA,PNA和HPA是外阴表皮良性或恶性增殖的一种好的组织标志物。
Objective To demonstrate the relationship between the changes in the cell surface carbohydrate residues and benign or malignant proliferation of vulvar epithelia. Methods A panel of twelve biot- inylated lectins was applied to study the presence and distribution of membrane carbohydrate residues in vulvar benign and malignant lesions of 60 patients, and the streptavidin-peroxidase histochemical technique was adopted. Results Lectins MPA, ConA, WGA and RCA showed no changes in staining distribution from normal vulvar epithelium to vulvar squamous cell carcinoma (VSCC). Lectins DBA, LTA, SBA, BPA and LFA were negative in all specimens. Only UEA-1, PNA, WA, DSA , HPA, SSA, and SNA changes were observed. UEA-1, PNA and HPA staining was negative for the basal cells in normal epithelia, whereas in VSCC a positive staining of UEA-1 and PNA was observed. WA and DSA staining was negative in all normal epithelia, but varied staining intensity and distribution were observed in vulvar squamous cell hyperplasia, vulvar condyloma acuminata and VSCC. Conclusion Fucose has been regarded as a marker for vulvar terminally differentiated keratinocytes. GalNAc residues gradually appear with vulvar epithelial cytoplasmic differentiation. Tn and T antigens appear during vulvar cell malignant transformation. UEA, WA, PNA and HPA may be regarded as a good histochemical marker for benign or malignant proliferation of vulvar epithelia.
