摘要
[目的]探讨乳腺癌、癌旁和正常乳腺组织中PCNA、bcl-2与p53基因蛋白表达的差异及其在乳腺癌变中的意义。[方法]用免疫组化法检测46例乳腺癌、癌旁组织和33例正常乳腺组织中PCNA、bcl-2与p53基因蛋白表达。[结果]①PCNA指数:乳腺癌与癌旁组织差异无显著性;乳腺癌和正常乳腺组织(46.03±8.76与32.13±6.78),乳腺癌旁和正常乳腺组织(42.90±7.92与32.13±6.78)差异均有显著性(P<0.01)。②bcl-2基因蛋白表达分值:乳腺癌、癌旁和正常乳腺组织分别为:0.62±0.47,0.91±0.58,1.56±0.84;分值大小顺序呈表达上升趋势。③p53基因蛋白表达分值:乳腺癌、癌旁组织和正常乳腺组织分别为:2.21±0.96,1.86±0.87,0.87±0.57;分值大小顺序表达呈下降趋势。④在乳腺癌组织PCNA指数与bcl-2呈中度负相关(rs=-0.537),与p53呈中度正相关(rs=0.628),bcl-2与p53呈中度负相关(rs=-0.476)。[结论]乳腺癌的发生、发展过程中PCNA和p53分值增大,bcl-2分值变小,预示组织恶性程度可能增加,对判定乳腺组织的良、恶性及预后具有重要临床价值。
[Purpose] To explore difference of expression of PCNA, bcl-2 and p53 proteins among breast carcinoma, para-carcinoma tissue and normal breast tissue and its value in development of breast carcionoma. [Method] PCNA, bcl-2 and p53 proteins in carcinoma tissue and para-carcinoma tissue in 46 patients with breast cancer and in 33 samples of normal breast tissue were detected by immunohistochemical S-P method. [Results] ① PCNA-PI showed no significant difference between breast carcinoma and para-carcinoma tissue (P〉0.05), but significant differences were found between breast carcinoma and the normal breast tissue (46.03±8.76 vs. 32.13±6.78), and between para-carcinoma tissue and the normal breast tissue (42.90±7.92 vs. 32.13±6.78, P〈0.01). ② The expression score of bcl-2 was 0.62±0.47 in breast carcinoma, 0.91±0.58 in para-carcinoma tissue and 1.56±0.84 in normal breast tissue, with a increasing trend from cancer to normal tissue. ③ The expression score of p53 was 2.21±0.96, 1.86±0.87 and 0.87±0.57 in breast carcinoma tissue, para-carcinoma tissue and the normal breast tissue, with a decreasing trend from cancer to normal tissue. ④ PCNA-PI showed moderately negative correlation with bcl-2 (rs=-0.537) and moderately positive correlation with p53 (rs=0.628), and bcl-2, moderately negative correlation with p53 (rs=-0.476) in breast carcinoma tissue. [Conclusion] Increases of PCNA and p53, decrease of bcl-2 predict the tissue malignant potential during occurrence and development of breast carcinoma, which play an important role for diagnosis and prognosis.
