单侧唇腭裂患者上颌快速扩弓矫治生物力学效果的三维有限元研究 被引量:7

A Study on Biomechanical Effects of Rapid Maxillary Expansion on the Craniofacial Complex With UCLP by the 3D Finite Element Method

摘要 研究单侧唇腭裂患者快速扩弓矫治的生物力学机理对探索影响术后复发主要因素和指导临床手术规划有重要意义。基于CT断层扫描技术、CAD建模技术和有限元建模技术建立单侧完全性唇腭裂(UCLP)患者上颌复合体三维有限元模型,利用三维有限元应力分析法研究扩弓力在UCLP患者上颌复合体的传递模式、骨缝应力状态及颅颌面各骨块位移特性,并与前人研究结果进行对比。结果表明扩弓力通过牙齿沿牙槽骨、上颌骨、翼突、蝶骨向颞骨颅底传导,应力呈梯度递减并向上、向后传导,蝶骨是其阻抗中心,在上颌前缘、颧蝶交汇处及鼻额部等部位有应力集中,由于上颌骨不对称结构的影响,裂侧应力及位移较大;扩弓量的最大区是在牙槽区,向上逐渐递减,形成底边向下的三角形,计算结果与临床治疗结果较吻合。由此说明,蝶骨的翼板近颅底处是约束上颌扩弓最主要的因素,上颌前缘也受较大应力,如果医师能松解翼板及打断上颌前缘,临床将获得更大的扩弓量,上颌骨、鼻腔及颧骨扩弓后也将趋于稳定,其在保持期复发回缩的可能性将减小。
