开腹胆囊切除术药物经济学影响因素分析初探

Initial Study of the Influential Factors of Pharmacoeconomics of Open Cholecystectomy

目的:考察开腹胆囊切除术患者的药物经济学影响因素,为下一步研究在胆囊切除病种中成本-效用分析方法的应用做准备。方法:收集某医院2002-2004年的开腹胆囊切除术患者的费用构成和基本信息,用ex- cel和spss统计软件包,对各信息作回顾性分析。结果:共分析1 055例,女性患者稍多于男性,中年患者居多,平均住院天数为(14．66±6．27)d,平均总费用为(8 756±2 041)元;出院诊断类型复杂,最常见的疾病类型为,胆囊结石伴急(慢)性胆囊炎(39．6％)、急慢性胆囊炎(16．8％)和肿瘤(8．3％);老年患者的平均总费用最高,平均住院费用最高的是胆相关肿瘤疾病的治疗,药费比例最高的是急(慢)性胆管炎;青霉素类复方制剂使用频率最大,西药费占总费用的比重最大(相关系数为0．582),住院天数与总费用的相关系数为0.515。

Objective： To inspect the influential factor of pharmacoeconomics of open cholecystectomy for the application of cost-utility analysis method in future research. Method ： The composition of fee and essential information of open cholecystectomy were collected in a hospital from 2002 to 2004. The excel and spss softwares were used to make a retrospective analysis of all factors. Result： There were 1 055 patients in the analysis. The number of the females was slightly more than that of the males, and the middle-aged patients were the most and the senile patients spent the highest average total expense. The average hospitalized days were （14.66±6.27 ）d. The average total expense was 8 756±2 041 yuan. There were many diagnostic types of leaving hospital, and the commonest disease types were cholecystolithiasis accompanying acute（chronic） cholecystitis （39.6%） and acute（chronic） cholecystitis （ 16.8% ） and tumor（ 8.3% ）. The highest charges were those of tumor’s therapy and the highest proportion of medicine charges was that of acute （chronic） cholangitis. The highest frequency of used antibiotics was that of compound penicillin. In all the expenses, the proportion of the charges for Western medicine was the biggest. （ The correlation coefficient is 0. 582. ） The related variation of hospitalized days to the total expense was the biggest. （The correlation coefficient is 0. 515. ）
