70岁以上老年患者非体外循环与体外循环冠状动脉旁路移植术68例 被引量：1

摘要 目的比较70岁以上老年患者行非体外循环冠状动脉旁路移植术(off-pum p CABG)与体外循环冠状动脉旁路移植术(on-pum p CABG)的临床效果。方法68例70岁以上老年患者,按手术方式不同分为两组,off-pum p组(40例),在非体外循环下行冠状动脉旁路移植术;on-pum p组(28例),在体外循环下行冠状动脉旁路移植术。结果两组各死亡1例,off-pum p组中1例死于术后急性肾功能衰竭,on-pum p组中1例死于肺部感染导致多器官功能衰竭。off-pum p组的移植血管数、术后使用正性肌力药物时间>24h、机械通气时间>24h、主动脉内球囊反搏(IABP)和二次开胸止血的比率均少于或低于on-pum p组(P<0.05);术后肺部感染、心房颤动、伤口并发症的发生率、住ICU和住院时间低于或短于on-pum p组(P<0.05)。off-pum p组随访36例,on-pum p组随访24例,随访时间3个月至2年,两组患者均存活,心绞痛症状消失。结论对70岁以上的老年冠心病患者行off-pum p手术,有手术创伤小、死亡率低、术后并发症少和安全有效等优点。
