摘要
目的:观察益母草对失血性休克大鼠血流动力学的影响。方法:实验于2005-05/07在河北北方学院医学院病理生理教研室完成。益母草注射液主要成份益母草碱,1mL注射液中含益母草5g,由深圳联合制剂公司生产。选用Wistar雄性大鼠40只,随机数字法选取16只大鼠用于微循环观察和器官微区血流量测定,剩余24只随机数字法分为正常组、模型组、生理盐水组和益母草注射液组,每组6只,用于血液流变学观察。麻醉后静脉注射肝素钠(700U/kg)全身抗凝,颈动脉插管放血,血压稳定30min后,放血至血压为33.9mmHg,造成失血性休克模型,益母草组(30mL/kg)按10.0g/kg以生理盐水稀释后静脉给药,生理盐水组给予等量的生理盐水,观察肠系膜微血管的一级细动脉、一级细静脉以及回肠下段肠系膜微淋巴管的口径和流态变化,测定不同器官固定部位微血流量和血液流变学,观察益母草注射液对失血性休克转归时血流动力学的调节作用。结果:40只大鼠纳入结果分析。①失血性休克时,微循环明显障碍,表现为微血管收缩、微血流变慢和血细胞聚集,经益母草注射液治疗后,微血管明显扩张,其流态和血细胞聚集积分值分别为1.02±0.32和1.12±0.29,生理盐水组流态和血细胞聚集积分值分别为1.82±0.42和1.89±0.34,差异有显著性意义(P<0.05)。②益母草注射液组和生理盐水组胃、肠、肝的微区血流量均得到改善,但益母草注射液组效果优于生理盐水组(P<0.05)。③益母草注射液治疗后,血黏度、血小板黏附率和聚集率明显低于生理盐水组,且红细胞变形能力明显增强(P<0.05)。结论:益母草注射液能明显改善失血性休克时的血流动力学异常。
AIM: To explore the effect of leonums heterophyllus solution (LHS) on hemodynamics in rats with hemorrhagic shock.
METHODS: The experiment was conducted in Department of Pathophysiololgy, School of Medicine, Hebei North University from May to July 2005. The major component of LHS (provided by the Shenzhen Unit Pharmaceutical Company) was leonurine, and 1 mL solution contained 5 g leonurus. Sixteen male Wistar rats of the 40 ones were randomly selected for microcirculation observation and determination of blood flow volume of organ micro-areas, and the rest 24 rats were randomly divided into normal group, model group, normal saline group and LHS group with 6 rats in each group for hernedynamic observation. The rats were given intravenous injection of hepariri sodium (700 U/kg) for body anticoagulation, followed by blood loss from carotid until the blood pressure of 33.9 mm Hg 30 minutes after stability of blood pressure to make the models of hemorrhagic shock; the rats in the LHS group (30 mL/kg) were intravenous injected with the solution diluted with saline by 10.0 g/kg; the rats in the saline group were given the same dose of saline. The caliber and hemedynamic changes of mesenteric microvascular primary arterioles and secondary venulae, and mesenteric microlymphatic vessels of the lower segment of ileum were observed to measure the microhemorheology and hemedynamic changes of the fixed sits of different organs and observe the regulative effect of LHS on the hemedynamics of hemorrhagic shock.
RESULTS: Totally 40 rats were involved in the results analysis. ①The microcirculation disturbance was mainly showed by micro-vessels contraction, slow micro-bloed flow and blood cells aggregation after hemorrhagic shock. After injection of LHS, the micro-vessels were obviously expended and its flow volume and the integral values of red blood cells had significant differences compared with the saline group [(1.02±0.32,1.12±0.29), (1.82±0.42, 1.89±0.34), P 〈 0.05]. ②The micro-aroa blood flow volume of stomach, intestine and liver was evidently increased in the LHS and saline groups, however, the former effect was superior to the latter (P 〈 0.05). ③After treatment of LHS, the blood viscosity and platelet aggregation were obviously lower than that of the saline group, meanwhile, the deformity ability of rod blood cell was obviously increased (P 〈 0.05).
CONCLUSION: LHS can obviously improve the abnormity of hemodynamics in rats with heamoregic shock.
