胎粪吸入综合征患儿支气管肺泡灌洗液中一氧化氮和内皮素水平变化及意义 被引量:1

The Concentration Change of Nitric Oxide and Endothelin-1 in Bronchoalveolar Lavage Fluid in Infants with Meconium Aspiration Syndrome

摘要 目的了解胎粪吸入综合征(MAS)患儿支气管肺泡灌洗液(BALF)中一氧化氮(NO)和内皮素-1(ET-1)水平变化,探讨胎粪吸入综合征肺部损害的发病机制。方法29例胎粪吸入综合征患儿于出生后1d内、出生后3d、撤机前(约4~7d,平均5.1d)取灌洗液,离心后取上清液测定NO及ET-1的含量。结果随着病程进展BALF中NO浓度逐渐升高,ET-1浓度逐渐降低,不同时间点两指标间差别均有显著性意义(均P<0.05)。结论NO的降低,ET-1的升高可能与MAS早期肺部病理生理变化有关,可能为导致肺血管过度收缩使肺动脉压力升高的原因之一。 Objective To understand the concentration change of nitric oxide (NO) and endothelin - 1 ( ET - 1 ) in bronehoalveolar lavage fluid (BALF) of the infants with meeonium aspiration syndrome (MAS) in order to give some scientific evaluation for clinical to treat infants with MAS. Methods Bronchoalveolar iavage was performed on the first day, the third day and before stopping the mechanical ventilation (nearly five days). Nitriate recovery and radio -immunity were used to detect NO and ET-1 concentration. All the tests were measured according to the manufacture's instructions. SPSSI l. 5 software was used to deal with the data. Results In BALF, the concentration of NO decreased and ET - 1 increased. There was significant difference between the indexes of different time ( P 〈0. 05). Conclusion During the early period of MAS, NO decreasing and ET - 1 increasing may be associated with hypoxemic and pulmonary hypertension.
