彩色多普勒超声诊断川崎病冠状动脉病变94例分析

Color doppler imaging diagnosis of Kawasaki disease with coronary artery lesions:analysis of 94 case

目的探讨多普勒超声心动图在川崎病冠状动脉病变诊断及随访中的作用。方法回顾性分析94例确诊的川崎病患儿的多普勒超声心动图表现。最长随访时间为8年。结果在受检的94例川崎病患儿中有冠状动脉病变共26例,3例发生冠状动脉瘤,左冠状动脉比右冠状动脉更易发生扩张性病变。随访结果表明,轻度扩张性病变较易恢复正常。1例冠状动脉瘤持续存在;1例动脉瘤回缩后主干轻度狭窄。结论多普勒超声心动图对于早期诊断川崎病冠状动脉病变,及时给予正确治疗,减轻冠状动脉的损害有非常重要的意义。有冠状动脉病变者均需定期随访,以指导用药及判断预后。

Objective To investigate the value of Doppler echocardiography in the diagnosing and following-up of the coronary artery lesions in Kawasaki disease. Methods Doppler echocardiography feature of 94 patients with Kawasaki disease were analysed retrospectively. The longest period of follow-up was 8 years. Results there were 26 patients with coronary artery lesions in total 94 patients with Kawasaki disease. 3 patients had coronary aneurysm. The lesion of LCA were more common than RCA.The follow-up resuhs revealed that mild dilatation formation recovered asily. 1 patient of coronary artery aneurysm persisted and 1 patient had slight stenosis in the left main coronary artery after coronary aneurysm drawn back. Conclusion Doppler echocardiography is of great values in diagnosing Kawasaki disease at early stage and plays an importam role in alleviating severity of coronary artery injury with early treatment.It is neeessary to perform long-term follow-up in patients with coronary artery lesions.
