恶化型脑梗死超早期临床与影像学研究

Ultra-early clinics and image of deteriorating massive cerebral infarction

目的探讨恶化型脑梗死的早期影像学诊断与治疗。方法32例临床拟诊为恶化型脑梗死患者行急诊CT、MR I弥散加权成像(DW I)检查。确认病例立即行rt-PA溶栓等治疗。回顾性研究上述病人的临床资料。结果最终确诊恶化型脑梗死25例。急诊CT检出大面积脑梗死14例,DW I检出25例。经溶栓等治疗,好转14例,死亡11例。结论DW I可准确地诊断恶化型脑梗死,超早期行溶栓等治疗效果较好。

Objective To explore ultra-early image and treatment of deteriorating massive cerebral infarction. Methods Thirty - two cases diagnosed probably with deteriorating massive cerebral infarction were given CT and MRI（DWI） examination. The diagnosed patiants were immediately accepted rt-PA therapy and etc. The above information were studied retrospectively. Results Twenty - five patients were diagnosed with deteriorating massive cerebral infarction. Emergency CT and DWI found massive cerebral infarction 14 and 25 cases respectively. With thrombosis resolution and other therapies,14 patients looked up,and 11 died. Conclusions With DWI, diagnosis can be made exactly in the uhra-early stage of deteriorating massive cerebral infarction. Ultra-early thrombosis resolution is effective for treatment of deteriorating massive cerebral infarction.
