关节内灌洗透明质酸钠治疗骨关节炎

The Treatment of Osteoarthritis (OA) with Intra-articular Injection and Irrigation of Hyaluronate

目的观察关节内灌洗复合透明质酸钠注射治疗膝关节骨性关节炎的效果。方法对38例膝关节骨性关节炎患者,首先用乳酸钠林格氏注射液冲洗关节腔,然后注射透明质酸钠,每周一次,连续4次为一个疗程。在治疗前和治疗后第9周,对患者进行体征检查及问卷调查,评价患者治疗前后的基础临床症状和日常活动能力的改善情况。对有序分类资料采用Wilcoxon配对秩和检验、对计量资料采用配对t检验进行统计学处理。结果患者采用疗法治疗前后静息痛、行走痛、上下楼梯痛、伸屈痛、压迫痛、关节肿胀、髌骨撞击、10min步行、下蹲拾物和端坐地面的评分,经Wilcoxon配对秩和试验,均P<0.01;滑液渗出量和关节伸屈度经配对t检验,P值亦均<0.01。结论该疗法对改善膝关节骨性关节炎患者的基础临床症状和日常活动能力效果非常显著。

Objective To investigate the effects of irrigation and injection of knee joint with hyaluronate in osteoarthritis （OA）. Methods 38 cases of knee OA were treated with intra-articular hyaluronate injection after lactate-linger solution irrigation once a week for 4 weeks as a course. Compare the results before and 9th week after treatment, the patients were asked to answer the questionaire of the basic clinical symptoms and daily living activities and were followed-up by physical examinations. Results Before and after treatment, the scores of silent pain, walking pain, go up and down stairs pain,extension flexion pain, compression pain, swelling of knee joint, 10 minutes walking,go up and down stairs ,patella impaction, pick up things and sit on ground, had shown very significant statistical differences （P〈0. 01）. Conclusion The intra-articular hyaluronate treatment improves knee joint of OA in basic clinical symptoms and activities of daily living significantly.
