原发性甲状腺恶性淋巴瘤的CT表现

CT Findings of Primary Thyroid Lymphoma

目的评价CT对原发性甲状腺恶性淋巴瘤(PTL)的诊断价值。资料与方法回顾性分析6例PTL的CT表现,并与手术及病理结果对照。6例均为女性,平均年龄65岁。结果6例均有近期发现颈部迅速肿大病史,5例伴桥本甲状腺炎。CT显示单发结节型1例,多发结节型1例,弥漫肿大型4例;6例肿瘤密度均匀,强化不明显,1例侵犯周围组织,2例出现颈部淋巴结受累。结论对于年龄>60岁的女性患者,有桥本甲状腺炎病史且近期甲状腺突然肿大者,CT表现甲状腺弥漫性肿大,密度均匀,强化不显著,应考虑PTL。

Objective To evaluate the diagnostic value of CT for primary thyroid lymphoma（PTL）. Materials and Methods CT findings of 6 patients with primary thyroid lymphoma were retrospectively reviewed. 6 patients were all female, The mean age of patients was 65 years. Results 6 patients were found as a rapidly enlarging thyroid mass with homogeneous hypodensity. 5 patients with Hashimoto＇s thyroiditis. 1 patient had a solitary nodule in thyroid tissue, 1 patient had multiple nodules in the thyroid, and 4 patients had a diffuse goiter. 6 patients had tumors with homogeneous density and low enhancement, 1 patient infiltrated the adjacent structures and 2 cases were revealed metastatic lympadenopathy on the neck. Conclusion It suggest the possibility of PTL that old female （ 〉 65 years） with a history of Hashimoto＇ s thyruditis had a homogeneous and rapidly enlarging thyroid mass, while the CT findings of PTL is usually manifested as larger mass with homogeneous density and low enhancement.
