摘要
目的:探讨犬右肺静脉脂肪垫对心房及右上肺静脉心房纤颤(简称房颤)诱发的影响。方法:19只犬麻醉后经右侧剖胸消融去除前壁脂肪垫、下腔静脉和左心房之间脂肪垫,颈部离断双侧迷走神经。分别在未刺激、刺激和消融右肺静脉脂肪垫的情况下,观察心房及右上肺静脉有效不应期和房颤诱发变化。结果:19只犬中2只死于心室纤颤,其余17只顺利完成实验。与未刺激相比,刺激右肺静脉脂肪垫后高位右心房、右心房游离壁和右上肺静脉有效不应期缩短,各部位房颤均易诱发,同时,心房有效不应期离散度增加。右肺静脉脂肪垫消融后各部位有效不应期及房颤诱发与未刺激时相比无差异。结论:刺激右肺静脉脂肪垫促进了房颤的诱发。
Objective : To investigate the effects of the fat pad at the right pulmonary vein-atria junction RPV fat pad) on atrial fibrillation (AF) induction of canine atria and the right superior pulmonary vein RSPV). Methods : Radiofrequency ablation (RFA) of the fat pad at the superior vena cava and aortic root(SVC-AO fat pad) and the fat pad at the inferior vena cava-left atria junction( IVC-LA fat pad) were carried out in 19 dogs after a right thoracotomy. Cervical vagus were isolated and cut. Changes of the effective refractory period (ERP) , AF induction of the atria and the RSPV were studied before and during RPV fat pad stimulation and after ablation of the fat pad. Results : RPV fat pad stimulation decreased the ERP of the high right atrium (HRA) ( P 〈 0.01 ) , right atrial free wall (RAF) ( P 〈 0.05 ) and the RSPV ( P 〈 0.01 ). RPV fat pad stimulation increased the dispersion of the atrial ERP ( P 〈 0.01 ) and the incidence of AF. Conclusion : RPV fat pad stimulation facilitated induction of AF.
