消化疾病患者血清食物过敏原IgG检测的临床意义 被引量:20

摘要 目的:研究消化疾病患者对食物过敏原的变态反应状况,为临床提供诊治相关疾病的依据。方法:采用ELISA法检测191例消化疾病患者血清中14种食物过敏原IgG浓度,按疾病分组观察过敏程度,并比较血清检测结果。结果:191例消化疾病患者血清食物过敏原IgG检测结果阳性率依次为:蟹55.5%,虾47.1%,大豆46.6%,小麦46.1%,牛奶35.1%,鸡蛋20.9%,大米17.8%,鳕鱼15.2%,牛肉14.7%,番茄10.5%,玉米9.4%,鸡肉8.9%,猪肉8.4%,蘑菇1.0%。检测结果显示,这些病例中,以蟹、虾、大豆和小麦的致敏性最高(阳性率与阴性率比较均无差别(P>0.05);牛奶的致敏性次之(阳性率与阴性率比较差别显著P<0.01);鸡蛋、大米、鳕鱼、牛肉、番茄、玉米、鸡肉、猪肉、蘑菇的致敏性较低(阳性率与阴性率比较差别非常显著P<0.001)。结论:血清食物过敏原IgG检测可以及时发现消化疾病患者对哪些食物过敏,并尽早将这些食物从患者食谱中排除,此为降低消化疾病患者食物过敏患病率和减轻相关症状的重要措施之一。 Objective :To investigate the allergization to 14 kinds of food allergens in serum of 191 patients with diseases of digestive system in order to provide a diagnostic bases for clinical diagnosis and treatment of the diseases. Methods: ELISA method was applied to determining the serum concentration of 14 kinds food allergen IgG in 191 hospitalized patients with diseases of digestive system. The degrees of allergiza- tion were observed and compared with sub-grouped diseases. Results:The positive rates of 14 food allergen IgGs in serum of 191 patients with diseases of digestive system were as follows:crab 55.5% , shrimp 47.1% , soybean 46.6% , wheat 46.1% , milk 35. 1% , heneggs 20.9% , rice 17.8% ,ling 15.2% , beef 14.7 % , tomato 10.5 % , corn 9.4 % , chicken 8.9% , pork 8.4% , mushroom 1.0%. The results showed that the highest allergization was found in soybean, wheat, crab, and shrimp without a great difference between the positive and negative rates ( P 〉 0.05 ) ;the allergization of rice, beef,ling and henegg was secondary to the above with a significant difference between the positive and negative rates (P 〈 0.05 ) ; corn, mushroom, tomato, chicken and pork were of low allergization with a very significant difference between the positive and negative rates (P 〈 0. 001 ). Conclusion:Determination of food allergen IgG in serum would helpful to promptly detecting the particular food allergizable to patients with diseases of digestive system. And eliminating these allergized food was one of the important therapeutic measures for reducing the incidence of diseases of digestive system and relieving their symptoms as well.
