颅脑外伤合并严重鼻出血的鼻科处理

The treatment of severe nose bleeding with head trauma in rhinology

目的探讨颅脑外伤合并严重鼻出血的部位及鼻腔处理。方法回顾性分析1998年10月～2004年10月,我院ICU收住的20例颅脑外伤合并严重鼻出血而邀请我科作鼻腔处理的患者的临床资料。结果20例患者中,作单侧鼻腔填塞的7例,作双侧鼻腔填塞的有11例,作单侧前、后鼻孔填塞的有2例;死亡3例。结论颅脑外伤合并严重的鼻出血时,在明确出血部位的前提下作相应的鼻腔填塞,仍有应用价值,但并发严重的颅内出血时,则无应用价值。

Objective To discuss the site of severe nose bleeding with head trauma and relevant treatment. Methods During the period of Oct. 1998 to Oct. 2004, clinical data of 20 cases of severe rhinorrhagia with head trauma in rhinology in ICU were analyzed retrospectively. Results 7 of 20 cases received unilateral nasal cavity packing. 11 cases received bilateral nasal cavity packing and 2 cases received unilateral anterior-posterior nasal cavity packing. 3 cases were dead. Conclusion Nasal cavity packing has clinical values in the event that the site of severe rhinorrhagia with head trauma can be located. But cases with severe intracranial hemorrhage shouldn＇t receive nasal cavity packing.
