多普勒超声仪对透析病人首次内瘘穿刺部位定位的研究

A study on Doppler ultrasound equipment to locate puncture area for dialysis patients with internal fistula for the first time

[目的]研究应用多普勒超声仪对提高动静脉内瘘穿刺成功率,减少内瘘血管并发症的效果。[方法]用随机数字表将首次使用内瘘的血液透析病人69例分为实验组35例和对照组34例,对照组用传统方法穿刺,实验组在德国DWL公司MDP型多普勒超声仪引导下穿刺。[结果]实验组静脉瘤样扩张、假性动脉瘤、针眼渗血、穿刺失败发生率低于对照组,针眼止血时间短于对照组。[结论]多普勒超声仪引导穿刺是提高穿刺成功率,减少血管并发症的有效手段。

Objective： To study on the achievement ratio of the first time internal fistula puncture by using Doppler ultrasound equipment, and its effect on reducing vascular complications of internal fistula. Method： A total of 69 patients underwent hemodialysis who was diagnosed internal fiseula for the first time were divided into test （35 cases） and control group （34 patients） according to random digits table. Patients of control group were punctured in traditional way. While patients of test group were punctured guided by MDP type of Doppler ultrasound equipment （made in DWL company of Germany）. Result： In terms of items including phlebangioma- like dilation, false aneurysm, pinpoint errhysis, failure of acupuncture and hemostasis time, patients of test group were superior to that of control group. Conclusion： Doppler ultrasound equipment guided puncture is an effective way to raise the achievement ratio of puncture and to decrease vascular complications for dialysis patients with internal fistula.
