胰岛素瘤的诊断和治疗

Diagnosis and treatment of insulinoma

目的 探讨胰岛素瘤诊断方法和治疗效果。方法 回顾性分析胰岛素瘤诊治资料。结果 23例中男9例，女14例。年龄19～69岁，40～50岁居多。病史最长10年，最短8个月。肿瘤位于胰头6例，胰体5例，胰尾8例，多发性3例，异位于十二指肠降部旁1例。肿瘤直径最小1cm，最大10cm。良性20例，恶变3例。行肿瘤摘除术19例，肿瘤加部分胰腺组织切除术2例，胰体尾切除术2例。术后发生胰漏3例，全部治愈。结论 胰岛素瘤诊断一旦成立，早期手术效果良好。

Objective To explore the diagnostic method and therapeutic effect of insulinoma.Methods The clinical data of insulinoma were analyzed retrospectively. Results In 23 cases of insulinma, male was 9, female 14, age from 19 -69, diseased history from 8 months to 10 years. Tumor at head of pancreas was in6 cases, at body of pancmas in S , at tail of pancreas in 8, multiple tumor in 3, ectopic pancreas at 2nd part of duodenum in 1 case, tumor diameter from 1 cm to 10 cm. Benign tumor was in 20 cases, malignant change, in 3. Tumor was excised in 19 cases, a part of pancreas tissue was excised in 2, pancreatic body-tailectomy in 2. Temporary pancreatic leak was in 3 cases and all of them were cured. Condusion Once insulinoma is diagnosed, operative therapy may get good result.
