老年晚期乳腺癌新辅助化疗后再手术的临床研究

<TITLE>CLINICAL ANALYSIS OF PRE-OPERATIVE NEOADJUVANT CHEMOTHERAPY IN ELDERLY PATIENTS WITH ADVANCED BREAST CANCER

目的 探讨老年晚期乳腺癌经新辅助化疗后的临床效果。方法对23例老年晚期乳腺癌患者行新辅助化疗．手术前采用TA方案化疗2—3个疗程，再行手术治疗。结果 临床观察完全缓解（CR）0例，部分缓解（PR）8例，轻度缓解（MR）11例，无变化（NC）4例，总有效率为82．6％。病理检查可见肿瘤细胞有不同程度变性坏死，间质纤维组织增多，炎性细胞浸润。结论 新辅助化疗能大量杀伤肿瘤细胞，可使肿瘤缩小，降低临床分期，为手术创造有利条件，从而改善预后。

Objective To investigate the effect of elderly patients with advanced breast cancer after pre-operative neoadjuvant chemotherapy. Methods A total of 23 patients with advanced breast canc：er underwent the TA neoadjuvant chemotherapy for 2-3 cycles hefore operation. Results The total response rate was 82.6% （19/23）, including no change （NC, n=4）, partial response （PR, n=8）, mild response （MR, n=11）, complete response （CR, n=0）. Pathologic changes of neoplasm, such as degeneration, necrosis, fibrosis, inflammatory infiltration, were observed after neoadjuvant chemotherapy. Conclusions Pre-operative neoadjuvant chemotherapy in the treatment of advanced breast cancer may destroy large number of tumor cells vigorously, diminish the primary lesion, make the tumor stage down, increase curative resection and long term survival rate.
