“慢性肾衰基本方”对肾小管-间质纤维化防治作用的实验研究

Effect of "chronic kidney failure basic prescription"(慢性肾衰基本方) on tubulointerstitial fibrosis in CRF rat

目的:研究“慢性肾衰基本方”的拮抗肾小管-间质纤维化(TIF)作用。方法:5/6肾切除大鼠模型给药4周,检测动物血尿素氮(BUN)和血清肌酐(Scr)水平;应用HE染色及PASM染色结合半定量计分法评估肾小管-间质损伤程度;应用免疫组化染色结合数码医学图象分析系统定量评判肾小管-间质α-平滑肌肌动蛋白(α-SMA)、角蛋白18(CK-18)、胶原I(Col-I)和纤连蛋白(FN)的表达量。结果:CRF模型未治疗组BUN、Scr水平显著较假手术组升高,肾小管-间质严重纤维化,α-SMA表达量显著上调而CK-18表达量显著下调,Col-I和FN表达量也显著上调。经"慢性肾衰基本方"治疗后CRF大鼠BUN、Scr水平显著下降,TIF指数降低,肾小管上皮细胞表达CK-18回升,Col-I、FN及α-SMA表达也有不同程度回降。结论:“慢性肾衰基本方”可通过抑制肾小管上皮细胞转分化为肌成纤维细胞、抑制肾小管-间质ECM合成而防治TIF。

Objective： To explore the effects of ＂chronic kidney failure basic prescription＂ （慢性肾衰基本方） on tubulointerstitial fibrosis（TIF）. Methods： 5/6 nephrectomized CRF rats were treated with ＂chronic kidney failure basic prescription＂ for 4 weeks. The levels of BUN and Scr were measured by automatic biochemistry analyzer. The tubulointerstitial injury index was also calculated using semi-quantitative analyzed methods after HE and PASM chemical dying. The expression of α-smooth muscle actin （α-SMA）, cytokeratin 18 （CK-18）, fibronectin （FN） and collagen I （Col-I） in tubulointerstitial region were quantitative analyzed respectively by motic digital medical image-analysis system after immunohistochemiscal analysis. Results： The levels of BUN and Scr as well as the TIF index were higher significantly in CRF model untreated group than sam-operating group. Furthermore, the expression of α-SMA as well as Col-I and FN were significantly increased, while the expression of CK-18 were decreased significantly in CRF model untreated group. The levels of BUN and Scr and TIF index in CRF model therapy group were significantly decreased compared to untreated group. The expression of α-SMA as well as Col-I and FN were significantly decreased, while the expression of CK-18 increased significantly compared to untreated group. Conclusion： ＂chronic kidney failure basic prescription＂ prevent phenotypic transdifferenciation of renal tubular epithelial cells and extracellular matrix accumulation in tubulointerstium effectly to alleviate TIF in CRF.
