微侵入DHS固定技术在老年股骨粗隆间骨折中的应用 被引量:2

摘要 目的:评价微侵入DHS固定技术治疗老年股骨粗隆间骨折的价值。方法:回顾分析老年股骨粗隆间骨折75例,其中30例行微侵入DHS内固定技术(MIDHSO),45例行切开复位DHS内固定技术(ORDHSO),对二组进行随访,比较术中,术后一般情况及并发症,并按Harris髋关节评分标准进行功能评价。结果:MIDHSO组与ORDHSO组相比,手术切口短,手术时间短,平均输血量少,住院时间短,并发症少,骨折愈合时间短,但髋关节功能优良率相似。结论:MIDHSO具有手术时间短、操作简便、手术创伤小等优点,是治疗老年人A1、A2型股骨粗隆骨折的首选方法。 Objective: To evaluate the value of mininally invasive DHS osteosynthesis(MIDHSO) in the treatment of intertrochanteric fractures in the elderly. Methods:75 cases of intertrochanteric fractures were retrospectively analysed. Of these, 30 cases were treated by MIDHSO and 45 with open reduction DHS osteosynthesis (ORDHSO). All cases were followed up, compared in general condition and complication of intraoperation and postoperation, and evaluated with the joint function according to Harris hip joint evaluation criteria. Results:No differences between groups were found in the distribution of sex and fractures type, age of patient and followoup duration. But the MIDHSO group had significant advantages in small operative cut, short operative time, little average blood transfusion, short hospital duration, little complication and short fracture healing time over the ORDHSO group, however, the excellent rate of hip joint function is similar. Conclusion:The MIDHSO enjoys the advantages in operative convenience and mininally invasive. It should be the first selected method for management of A1 or A2 type intertrochanteric fractures in aged patients.
