后颅窝区脑膜瘤的显微外科治疗126例报告

MICROSURGERY ON POSTERIOR FOSSA MENINGIOMAS: A REPORT OF 126 CASES

目的 探讨后颅窝脑膜瘤的显微手术方法及效果。方法 回顾性分析显微外科治疗的126例后颅窝脑膜瘤的临床资料。结果 肿瘤全切29例，近全切63例，部分切除34例，术后γ-刀87例，术后神经症状改善43例，无改善49例，加重及出现新的颅神经障碍31例，死亡10例。随访3个月。12年有11例肿瘤复发。结论 后颅窝脑膜瘤全切率低，术后并发症多，死亡率高，根据部住不同，选择合适的手术入路，能提高全切率，减少并发症，降低死亡人数，提高生活质量。

Objective To discuss microsurgical approaches and results of 126 patients with posterior fossa meningioma. Methods 126 patients from 1990 June to 2005 April were analyzed retrospectively with regard to operation approaches and long-term results. Results All patients were underwent operation for resection, 29 cases were total removed, 63 were subtotal removed, and 34 cases were partly removed. The postoperative neurological statuses were improved in 43 patients, remain unchanged in 49 and aggravated in 31. 10 deaths occurred. 92 patients were followed-up, 11 tumors were recurrent. Conclusion Appropriate approaches are selected according to the size of tumors and microsurgical procedure can lead to improvement in the patients＇ overall outcome and decreasing complications for posterior fossa meningiomas.
