CT图像计分与颅脑损伤预后的临床研究

Significance of CT image scores in judging prognosis on head injury

目的评价CT图像计分法对颅脑损伤分型和预后评价的实用性。方法对1042例颅脑损伤病人进行前瞻性研究。CT检查以颅底内面为基线向上连续平扫,层厚和层距均为10mm,依据损伤程度进行计分和分组处理。保守治疗组624例,手术治疗组418例,结合临床进行对照分析。结果本组中轻型颅脑损伤(≤5分)236例,占22.6%;中型(6￣10分)370例,占35.5%();重型(≥11分)436例,占41.8%。各型预后之间有统计学显著差异(P<0.05)。结论CT图像计分能较好地判断颅脑损伤的程度,为临床治疗和判断预后提供重要的客观依据。

Objective To explore the correlation of CT image scores and prognosis in head injury. Methods The patients of 1042 cases with head injury were prospectively studied with CT. All cases were scanned with 10mm slice thickness and interval. According to the CT scores, the compare group （624 cases） and operate group （418 cases） were found for therapy and prognosis. Result There were the mild injury （≤5 scores） 236 cases（22.6%）, middle injury （6-10 scores） 370 cases（35.5%） and severe injury （≥11 scores） 436 cases （41.8%）. There were obvious difference （p〈0.05） for the patient prognosis in different groups. Conclusions CT image scores can be used for the prognosis of head injury.
