三种辅助检查在大肠癌术后复发转移中的临床应用及评价 被引量:2

摘要 目的探讨符合线路显像、B超、CT在大肠癌术后复发转移中的临床应用价值并以评价。方法对22例临床怀疑复发转移的大肠癌术后患者行18F-脱氧葡萄糖(FDG)符合线路显像、B超及CT检查,计算其诊断准确性、灵敏度、特异性,并与再次手术的病理诊断或综合各项检查结果和(或)随访证实结果分别做比较。结果符合线路显像、B超及CT检查与最后诊断间的Kappa值分别为0.773、0.241、0.455,P值分别为0.020,0.031,0.500。结论18F-FDG符合线路显像对大肠癌术后复发转移的诊断与最后诊断间有较高的一致性,其诊断准确性明显高于B超和CT,灵敏度明显高于B超,有较高的临床应用价值,但应合理掌握。 [Objective] To discuss the clinical value of 18F-FDG SPECT/PET, UB and CT in the diagnosis of recurrent or metastatic tumor of colorectal carcinoma. [Methods] 22 cases of postoperative patients with tumor of colorectal carcinoma were checked with 18F-FDG SPECT/PET, UB and CT respectively, the results were analyzed by calculate its accuracy, sensitivity, specificity and also analyzed by SPSS 10.0 software compared with the ultimate diagnosis. [Results] The value of Kappa of 18F-FDG SPECT/PET, UB and CT were 0.773, 0.241, 0.455, respectively. [Conclusion] There is a good agreement with the ultimate diagnosis and 18F-FDG SPECT/PET, 18F- FDG SPECT/PET has high clinical value in the diagnosis of recurrent or metastatic tumor of colorectal carcinoma, but its should be used rationally. Combination of 18F-FDG SPECT/PET, UB and CT will make the result to be more satisfied.
