腹腔镜手术中转开腹13例分析

Analysis of 13 cases of conversion from laparoscopy to laparotomy

目的探讨有效降低腹腔镜手术中转率的方法。方法回顾性总结分析该院2000年11月～2004年10月911例腹腔镜手术中13例中转开腹的原因。结果该院中转率为1.43%,低于文献报道的平均值。该组术中止血困难、粘连、损伤各3例,各占23.1%;解剖变异2例,占15.4%;找不到纱条和可疑胆囊癌变各1例,各占7.69%。结论腹腔镜手术中转率的高低,与手术适应证的选择和术者的技术熟练程度有直接的关系。

[Objective] To disscuss the way of efficiently decreasing the rate that Caesarean operations are required in peritoneoscope operations. [Method] To summarize and analyze the reasons why our hospital took 13 cases out of 911 operations from November 2000 to April 2004. [Result] The rate that Caesarean operations are required in peritoneoscope operations in our hospital is 1.43%, below the average reported. In this group, the difficuhy of stanching bleeding during operations, adhesion, injure, each of them has 3 cases, taking up 23.1%; dissecting variation has 2 cases, taking up 15.4%; not finding gauze and guess gallbladder cancer, each has one case, taking up 7.69%. [Conclusion] In the peritoneoscope operations, it is directly related to the choice and the operator＇s skills whether the rate that Caesarean operations are required in peritoneoscope operations is high or not.
