肌层切开膈肌瓣覆盖加贲门瓣成形术治疗贲门失弛缓症13例

Treatment Of Cardia Achalasia With Diaphragmatic Valve Covering And Cardiovalvoplasty: A Report Of 13 Cases

我们在Ｈｅｌｌｅｒ手术基础上，增加贲门瓣成形和膈肌瓣覆盖成形的手术方法，治疗１３例贲门失弛缓症并取得良好效果。此术式可以从两个方面增强抗反流机制：一是贲门瓣成形起到瓣的作用；二是使带有良好血运及丰富神经纤维支配的膈肌覆盖食管下段肌层切开之缺损处，起到类似膈肌的钳闭作用。本文对手术方法，适应症及注意事项进行了讨论。

A modified procedure of Heller’s operation as cardiovalvoplasty with a diaphragmatic valve covering was carried out in our hospital from 1983 to 1995, 13 cases of cardia achalasia were treated and got good results. The anti- reflux mechanism would be strengthened in two ways: (1) The plastic cardiac valve may be served as a valve; (2) The diaphragmatic valve which is abundant in living nerves and vessels may cover the incision over the lower esophagus fitly obtaining a restraint efficacy similar to a diaphragm. The techniques, indications and cautions about surgery were also discussed.
