聚焦超声用于治疗慢性宫颈炎2646例的临床疗效分析

Clinical Study on the Chronic Cervicitis with Focused Ultrasound

目的探讨聚焦超声用于治疗慢性官颈炎的有效性和安全性。方法选择2004年8月～2005年7月，我院妇科门诊经妇科检查，细胞学及阴道镜诊断的慢性宫颈炎患者3976例进行聚焦超声治疗。病理改变全部为宫颈糜烂者721例，其中宫颈糜烂合并纳氏囊肿者1535例；合并宫颈肥大者1720例；患者年龄最小19岁，最大49岁，平均年龄33．82岁。根据宫颈糜烂程度、病理类型将慢性宫颈炎患者分类，聚焦超声治疗后3个月定期随访，根据超声治疗慢性宫颈炎的四级评分法，分析评价超声治疗慢性宫颈炎的有效性和安全性。结果随访到2646例，随访率65．5％；失访1330例，失访率33．5％。2646例患者中，痊愈：1561例（占58．99％）；显效1042例（占39．38％）；有效43例（占1．63％）；无效：0例。治愈率与糜烂面积及深浅程度有关（P〈O．05）。超声治疗后仅有10％的息者有出血的情况且绝大部分（83．4％）为少量的渗血；仅1．2％的患者会出现中等量的出血；愈合后官颈恢复正常生理结构形态，2646例治疗后宫颈均未见有疤痕形成。结论应用聚焦超声治疗慢性宫颈炎临床疗效确切，不良反应及并发症少，是一种值得推广的新方法。

Objective To investigate clinical efficacy and safety of focused ultrasound treatment for the chronic cervicitis. Methods There were 3976 patients with chronic cervicitis were treated by focused ultrasound in the hospital during Aug. 2004 to July 2005. The patients were follwed up in 3 months after the treatment. According to degree of cervical erosion, pathologic type and course of disease,2 646 patients with the chronic cervicitis were classified. Efficacy and safety of the focused ultrasound were evaluated after the treatment. Results Among 3976 treated patients,2646 responded to the following up. Among 2646 patients, 1 561 （58.99%） were cured, 1 042 （39.38%）were found much more improved, 43 （1.63 % ）were improved. Total efficiency rate of the treatment for chronic cervicitis was 100%. The total efficiency rate was associated with the area and the depth of the cervical erosion（P〈0.05）. There were few side effects such as plentiful exudates of vagina and bleeding of cervix. Conculion The chronic cervicitis can be treated by focused ultrasound therapy effectively and safely. There is little complication in the treatment. The focused ultrasound therapy will be an important therapy for the chronic cervicitis and has a good application prospect.
