非体外循环冠状动脉搭桥术病人的术中监护

摘要 目的探讨非体外循环不停跳冠状动脉搭桥手术(OPCABG)的监测与护理。方法配合麻醉师持续监测血液动力学各项指标,密切观察血压、心律、体温的变化,严格无菌管理,控制手术感染。在术中移动、抬高心脏发生循环功能紊乱时,应用血管活性药物,及时调整手术体位,进行有效的心血管功能支持。结果病人均在非体外循环心不停跳下完成冠状动脉搭桥手术。手术过程中血液动力学稳定,没有手术感染。结论0PCABG关键是维持血液动力学稳定,维持心肌氧供需平衡,避免加重心肌缺血。 Objective To probe into the monitoring and caring of patients in off-pump coronary artery bypass grafting (OPCABG). Method During the operation, hemodynemic parameters were monitored; patient's blood pressure, heart rate and body temperature were closely observed, Sterile principles were strictly followed to avoid postoperative infection, When circulative disorder occurred while moving or elevating the heart in operation, vasoactive agents were administered; and operation position was promptly adjusted to guarantee effective cardiovascular function, Result All the patients smoothly experienced the operation with stable hemodynemics. No postoperative infection was found. Conclusion It is ve(y important to maintain the stable hemodynemics and myocardial oxygen supply during (OPCABG) to avoid the increasing of myocardial ischemia.
