迟发性运动障碍临床特点及其相关因素分析

Analysis of the related factors and the clinical characteristics of the tardive dyskinesia

目的:探讨迟发性运动障碍临床特点及其相关因素。方法:选取我院三个病区的退伍军人精神分裂症病人295例,使用异常不自主运动量表(AIMS)评定患者有无TD及其严重程度,同时应用BPRS量表评价其精神症状。结果:TD发生率26.78%,好发部位依次为舌部、唇部和口周部、颌部、上肢、下肢、面部表情肌、颈、肩、臀。异常运动的严重程度主要为轻度(50.6%)和中度(43%);异常运动对正常运动的影响极轻到中度的病人之和只有21.5%,而大部分人没有影响(78.5%),能觉察到异常不自主运动的病人为32.9%,觉察不到的为67.1%。上肢异常不自主运动对正常运动的影响最重,且病人最易觉察到。TD的相关因素分析发现首发年龄越小、病程越长、合用安坦四肢不自主运动有加重趋势。结论:TD发生率较高,好发于舌部、唇和口周部,2/3的病人异常不自主运动对正常运动没有影响且觉察不到。

Objective ：To explore clinical characteristic and its correlation factor of tardive dyskinesia（TD）. Methods：Selects 295 cases retired serviceman inpatients with schizophrenia in my hospital three wards, The diagnosis and its the serious degree of TD was made according to the abnormal involuntary movement scale（AIMS） score, Meanwhile psychiatric symptoms were rated using the BPRS scale. Results： The incidence of TD was 26.78%. The predilection site is the tongue,lip and mouth,jaw,upper limb,lower limb, facial expression muscle ,neck shoulder buttocks in turn. The abnormal movement serious degree mainly is mild （50.6%） and moderate （43%）. The sum of influence of the abnormal movement to normal movement extremely light to the moderate patients is 21.5%, does not have the influence （78.5%） ,The patient who can perceive the unusual movement is 32.9%, did not perceive is 67.1%. The influence of upper limb abnormal involuntary movement to normal movement is the most serious, also the patient is the easiest to perceive. Conclusions： TD formation rate is high, The predilection site is the tongue,lip and mouth, 2/3 patients abnormal involuntary movement does not affect the normal movement, also does not perceive.
