Clinical study on Misoprostol's assistant effect on painless artificial abortion of early pregnancy

目的探讨无痛人工流产前、后舌下含化米索前列醇终止早孕的临床效果。方法将孕6～10周需终止妊娠并要求无痛人工流产的589例孕妇随机分为A组(198例)、B组(196例)和C组(195例)。A组在用丙泊酚及芬太尼行无痛人工流产前半小时及其后分别舌下含化米索前列醇0.2mg;B组舌下含化卡孕栓0.5mg;C组不用米索前列醇和卡孕栓。观察三组术中宫颈松弛扩张情况、宫缩幅度、术中出血量及术后阴道流血时间。结果A组宫颈松弛扩张情况明显好于B、C组;A组宫缩幅度明显大于B,C组(P<0.05);且易于手术操作,术中出血量、术后阴道流血时间均明显的小于或短于B,C组(P<0.01)。结论无痛人工流产前、后舌下含化米索前列醇可增强子宫收缩、手术易于操作、术中出血少、术后阴道流血时间短,费用低,值得临床推广应用。

Objective To study the clinical assistant effect of misoprostol an painless artificial abortion of early pregnancy. Methods A total of 589 women at 6 - 10 gestation weeks, who asked for painless artificial abortion, were randomly divided into group A （ n = 198）, group B （ n = 196） and group C （ n = 195）. Group A taken 0.2 rag of misoprostol sublingually half an hour before and after the painless artificial abortion with propofol and fentany lasanaesthetic and antalgic. Group B taken 0.5 mg of earboprost methylate sublingually half an hour before and after the painless artificial abortion. Group C underwent painless artificial abortion without misprostol or carboprest methylate, Laxation and dilatation of the cervix during the operation, uterine contraction, blood loss during the operation and postoperative bleeding time were observed. Results Group A achieved better affects in laxation and dilatation of the cervix than the other two groups. Uterine contraction was better in group A than group B and C （ P 〈0.05）, Easier to manipulate, less operative bleeding （ P 〈0.01） and shorter postoperative bleeding （ P 〈0.05） were obtained group A than group B and C. Conclusions Misoprostol used before and after painless artificial abortion may intensify uterine contraction and make operating easier. It helps to decrease operative blood loss and shorten the postoperative bleeding time. It is valuable to apply it clinically.
