摘要
目的探讨腹膜后血肿的诊断和手术时机的选择。方法收集1995—2004年腹膜后血肿58例进行回顾性分析。依据临床表现,体格检查、B超和CT等检查结果综合分析诊断,选择恰当的手术时机。结果本组58例中采用手术治疗42例,53例治愈,5例死亡。结论腹膜后血肿的诊断除依靠病史、体格检查外,腹穿、B超、腹部X线平片、CT检查均有利于诊断。对于中央区腹膜后血肿要积极探查,肾区后腹膜血肿选择性探查,大血管损伤伴后腹膜血肿多需探查,骨盆区后腹膜血肿应避免探查。
Objective To approach the diagnosis and operation timing of retroperitoneal haematoma（RH）. Methods The clinical data in 58 cases with RH were collected, analyzed retrospectively from 1995 to 2004. The operation timing was fixed in accordance with comprehensive diagnosis from combination of clinical features, uhrasonometry and CT scan, Results In all patients ,42 cases were performed procedure exploration , the remainder 16 cases were treated with other therapies. 53 cases survive, 5 died. Conclusions The peritoneum puncture, uhrasonometry B check, plain peritoneal X - ray and CT scan are available in the diagnosis of RH, besides disease history, physical examinations, The procedure exploration should be early performed in mid- area RH patients, cautiously done in postrenal area, commonly used in patients with the larger vascular injuries, but abdominal membrane hematoma exploratory should be avoided in pelvis area.
