前后联合手术入路治疗复杂髋臼骨折

Operative treatment of complex acetabular fractures by combined surgical approach

目的 探讨联合入路治疗复杂髋臼骨折及复位固定技巧。方法 对1999-2004年应用联合手术入路收治的36例复杂髋臼骨折的治疗进行回顾性研究。术后随访6个月-5年，平均3．6年。结果骨折复位情况按Mana评定标准，解剖复位2l例，满意复位11例，不满意复位4例。关节功能按照改良的Mefled’Aubigne and Postal评分系统进行评估，关节功能优26例，良6例，可4例，优良率88．9％。结论手术治疗复杂髋臼骨折时，联合入路能够很好地显露骨折部位，方便固定，临床效果满意。

Objective To evaluate the surgical treatment and technique of reduction and internal fixation in complex acetabular fractures through combined approach. Methods The clinical data of 36 cases of complex acetabular fractures treated by combined approach were studied retrospectively. The follow-up was 6 months - 5years. Results According to Matta standard,fractures were reduced anatomically in 21 cases,perfectly in 11 cases and poorly in 4 cases. The modified Merled＇Aubigne and Postel score system showed that the excellent results was found in 26 cases,good in 6 cases and fair in 4 cases. Conclusion Combined approach can expose fracture perfectly and fix conveniently in surgical treatment of complex acetabular fractures with excellent result.
