子宫肌瘤的腹腔镜手术

LAPAROSCOPIC SURGERY FOR HYSTEROMYOMA

于1995年元月～1996年8月对16例子宫肌瘤患者进行了腹腔镜手术；其中肌瘤剔除（LHM）9例，子宫次全切除（LISH）4例．子宫全切（LAVH）3例。除2例子宫切除因瘤体较大或创面渗血而中转开腹手术外．其余病例均顺利完成手术．未发生术中大出血及邻近脏器损伤。术后恢复快，住院时间短。文中对子宫肌瘤的腹腔镜手术的适应证及方法选择、手术技巧、术中并发症的防范等进行了讨论，认为只要掌握好手术适应证及具有熟练的腹腔镜手术技能，子宫肌瘤以腹腔镜进行手术的治疗效果明显优于开腹手术。

16 cases of laparoscopic surgery for uterus myoma were carried out in obstetrics and gynaecololgy department, Qiannan Autonomous Region People's Hospital from January 1995 to August 1996, among them, there were 9 cases of laparoscopic hysteromyomectomy (LHM), 4 cases of subtotal hysterectomy and 3 cases of complete total hysterectomy. Except for 2 cases were changed to the laparo-hysterectomy due to the larger tumors or haemorrhage oozed to the wound surface during the coutse of operation, all other laparoscopic operation, were very successful without major haemorrhage and impairing nearby organs. After operation, all cases recovered quickly and their hospitalazation days were signiffeantly reduced.This paper discussed the indication, selection of methods and skills for the operation and the protection against complications during the surgery, and considered that as far as one has a better knowledge of the indication and skillful techniques for the laparoscopiC surgery, the theraputical effectiveness of bysteromyoma ptovided by laparoscopic surgery would be superior to that by laparotomy
