基因重组活化凝血因子Ⅶ制品对肝移植术中凝血功能的影响 被引量:1

摘要 目的观察基因重组活化凝血因子Ⅶ(rFⅦa)制品对肝移植手术中患者凝血功能的影响。方法20例因终末期肝硬化行背驮式原位肝移植(OLT)的患者,随机分为试验组和对照组,分别于切皮前10分钟单次静注rFⅦa(70μg/kg)、生理盐水。并于注药前(T1)、注药后30分钟(T2)测定患者的凝血酶原时间(PT)、国际标准化比率(INR)、活化部分凝血激酶时间(aPTT)、纤维蛋白原、血小板。观察两组患者凝血参数的变化。结果与对照组相比,试验组的PT(P=0.026)、INR(P=0.029)和aPTT(P=0.032)在T2时段明显降低。而纤维蛋白原和血小板则无明显变化。结论rFⅦa能快速、有效地改善肝移植患者的凝血功能,纠正PT、INR和aPTT比值,进而减少患者术中出血。
