纤维内镜手术治疗脑室系统疾病

摘要 目的 应用Olympus CHFP-20纤维内镜对适应病例进行手术治疗,旨在弥补硬质神经内镜的部分缺陷。方法 应用纤维内镜对适应病例实施手术治疗:分隔性脑积水、透明隔囊肿行透明隔造瘘术;梗阻性脑积水行第三脑室底造瘘术。结果 本组27例,均手术成功,不良反应较轻。术后颅内压增高症状均得到缓解。追踪观察3~20个月,无症状复发者。结论 OlympusCHFP-20纤维内镜完全适合脑室系统手术需要,可顺利抵达病变部位,操作灵活可靠;显示图像能明确分辨出精细脑室结构;镜体柔软,适应脑室结构,损伤更小;因视野范围较大,可达到硬镜不能达到的区域进行观察、操作。其图像质量较硬镜稍差;其位置感与距离感亦差于硬镜,进行手术前应充分熟悉其性能。 Objective Doing surgical operations under fiberoptic endoscope of Olympus CHF P - 20,aim to make up some defects of stereoplasm endoscopes. Methods Did surgical operations under fiberoptic endoscope:separable hydrocephalus, pellucid septum fistulation to cyst of pellucid septal cave, fistulation of the basement of the third cerebral ventricle to obstructive hydrocephalus. Results Twenty - seven cases under operations were successful,with light adverse effect. And intracranial hypertension was relieved after operation. All patients were followed up 3 - 20 months, with no symptomatic recidivist. Conclusion Fiberoptic endoscope of Olympus CHF P - 2C is compete suitable to ventricular system operations,which can reach diseased region successfully with agility and reliability. Next,its display can identify subtle ventricle structure, and its soft shank suits to ventricle structure, with little damage. Meanwhile, because of its wider visual field, fiberoptic endoscope can arrive at region that stereoplasm endoscope cann't. Finally,the picture quality and the sense of position and distance of fiberoptic endoscope inferior to stereoplasm endoscope's so you must be familiar with its performance before operation.
