血浆BNP水平在围产期心肌病中应用

Clinical Evaluation of Plasma Levels of B-type Natriuretic Peptide in Patients with PPCM

目的观察血浆中B型钠尿肽(BNP)水平变化,研究其对围产期心肌病(PPCM)的诊断、治疗评价及预后判断方面的临床价值。方法应用酶联免疫法对26例PPCM患者治疗前后和25名正常围产期妇女对照者的血浆BNP水平进行检测,超声心动图测左室射血分数、心脏指数。结果PPCM患者血浆中BNP治疗前后比较差异有显著性(P<0.05),正常对照组与PPCM组治疗前比较差异有显著性(P<0.001),BNP水平与NYHA心功能分级呈良好正相关,与左心室射血分数、心脏指数呈良好负相关。结论BNP的检测有利于PPCM患者的早期诊断、疗效评价和预后评价有重要的意义,因此具有重要的临床应用价值。

Objective To investigate the value of brain natrluretic peptide （BNP） in the diagnosis , illness evaluation, and prognosis on the patients with PPCM. Methods BNP levels in the plasma of 26 patients with PPCM before and after treatment and 25 normal women as controls were detected by ELISA and radioimmunoassay. Results There were significant differences between the patients with PPCM before and after treatment in the levels of BNP （ P 〈0. 001 ） ,which was positively correlated with NYHA class , and negatively with LVEF and CI and difference also in PPCM group and control group （ P 〈 0. 05 ）. Conclusion The change of BNP levels are significant to the diagnosis, treatment and prognosis in the patients with PPCM. BNP is a useful biomarker for early diagnosis, ill- ness evaluation and prognosis of PPCM.
