应用伤害控制原则处理老年人浮膝骨折

Treatment of floating knee fracture in old patients with damage control theory

目的探讨应用伤害控制原则处理老年人浮膝骨折的方法。方法回顾性分析2001年2月～2004年8月60岁以上浮膝骨折住院治疗的病例21例应用伤害控制原则的处理。结果 参照Karistrom疗效评定标准，膝关节功能优良率达到78％。结论治疗开放性浮膝骨折需要应用伤害控制的观念，兼顾患者全面情况，同时治疗其他并存疾病。手术固定以髓内钉为主终末固定，并注重早期功能锻炼。

Objective To explore the treatment of floating knee fracture in old patients with damage control theory. Methods To review the 21 hospitalized aged cases of floating knee fracture with damage control from February, 2001 to August, 2004. Results Using Karistrom＇s knee scale,the total rate of good to excellent results was 78%. Conclusion The damage control theory is necessary to the treatment of open floating knee fracture. At the same time, the attention must be paid to the whole pathophysiological change of the patients and the treatment of other polytrauma or complications. In the operations of floating knee fracture,intramedullary nails are used for fixation of the fracture. After the operations,it is necessary to enhance the early training of the knee joint.
